Mounjaro Pen Won't Twist or Turn — What to Check First

Mounjaro is supplied as a pre-filled KwikPen designed for once-weekly injection. The pen requires a twisting action to advance the dose selector, and several things can interfere with that mechanism.

The pen is too cold

This is the most frequent reason a Mounjaro pen feels stiff or completely refuses to turn. The medication should be brought to room temperature before use — check the patient information leaflet that came with your pen for the recommended time, but many users find allowing the pen to sit at room temperature for around 30 minutes helps significantly. Never try to warm the pen in a microwave or hot water, as heat can damage the medication.

The needle is not attached correctly — or not attached at all

On some KwikPen designs, the twist mechanism will not engage until the needle is properly attached and locked. If the outer needle cap is only partially on, the pen may feel jammed. Remove the needle cap, check the needle is screwed on firmly, and try again.

The dose selector is already at the correct position

If the pen has already been set to the correct dose marker (typically position 1 before a first injection), it will not turn further in that direction. Check the dose window to see whether the indicator is already where it needs to be before assuming there is a problem.

The pen has been dropped or damaged

Physical impact can occasionally affect the internal mechanism. If the pen was dropped and the twist still does not work after trying the steps above, do not attempt to force it — contact your pharmacy or prescriber for a replacement.
