What Are the Main Warnings Associated with Mounjaro?
As a prescription-only medicine subject to additional monitoring, Mounjaro comes with several precautionary warnings outlined in its official product information. These are based on data gathered during clinical development and ongoing post-market surveillance.
Black Triangle Status (▼)
In the UK, Mounjaro carries a black triangle symbol (▼), meaning it is under additional monitoring to collect safety data as quickly as possible. This does not mean it is unsafe — many newer medicines carry this status. It does mean that if you notice any side effect, however minor, reporting it matters. You can do this through your national reporting scheme (such as the UK's Yellow Card scheme, or the equivalent programme in your country), and you should always mention side effects to your prescriber or doctor.
Possible Risk of Thyroid C-Cell Tumours
Studies in rodents identified a potential association with thyroid C-cell tumours. Whether this applies to humans is not yet established, but as a precaution, Mounjaro is generally not recommended for people with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). If you have any thyroid concerns, discuss them with your prescriber or doctor before or during treatment.
Pancreatitis
Cases of pancreatitis (inflammation of the pancreas) have been reported in people taking GLP-1 receptor agonists, including tirzepatide. Symptoms to be aware of include severe, persistent abdominal pain that may radiate to the back, sometimes accompanied by vomiting. If you experience this, contact a healthcare professional promptly — do not wait to see if it passes.
Gallbladder Problems
Rapid weight loss in general, and GLP-1 medicines specifically, have been associated with gallbladder issues including gallstones. Symptoms such as upper-right abdominal pain, nausea, or fever after eating should be discussed with your prescriber or doctor without delay.
Hypoglycaemia (Low Blood Sugar) When Combined with Other Medicines
On its own, Mounjaro has a low risk of causing hypoglycaemia. However, if you are also taking insulin or a sulphonylurea for diabetes, the risk increases. Your prescriber should advise on monitoring — never adjust your insulin or other diabetes medicines without guidance from your healthcare professional.
Mental Health and Mood Changes
Some people taking weight-loss medicines report mood changes. If you notice low mood, anxiety, or any thoughts of self-harm, speak to your prescriber or doctor straight away. This is a priority conversation, not one to defer.
For a full, current list of warnings and contraindications, you can consult the Mounjaro patient information on the electronic Medicines Compendium (eMC), or the leaflet that came with your pen.
It is also worth being cautious about where your supply comes from. If you have any concerns about the authenticity of your medication, read about the risks associated with unregulated Mounjaro supply — this is a growing safety issue. For general information about the medicine and how it works, the Mounjaro overview is a useful starting point.

How Tracking with GLP Trackr Supports Your Safety Awareness
Staying informed about warnings is only half the picture — the other half is noticing and recording what actually happens to you day to day.
GLP Trackr's side-effect tracking lets you log how you feel after each dose: nausea, abdominal discomfort, fatigue, mood shifts, and more. Over time, the app surfaces patterns — for example, whether symptoms cluster around a particular dose strength or timing — so you have something concrete and organised to share at your next appointment. A clear log is far more useful to your prescriber or doctor than trying to recall weeks of experience from memory.
The app's dose and injection logging also means your treatment history is always to hand. If a question about your schedule comes up during a consultation — or if you need to report a side effect and want to tie it to a specific dose — the record is already there.
GLP Trackr even includes a doctor report feature, which compiles your logged data into a summary designed to support those prescriber conversations. It is not a medical assessment — it is a structured record of your own journey.
If you are curious about the differences between Mounjaro and other formulations, the page on Mounjaro in tablet form covers what is currently known.
Remember: GLP Trackr is a tracking and journaling tool — it is not a substitute for clinical advice. If any warning symptom applies to you, contact your healthcare professional promptly. Any decision about continuing, pausing, or adjusting your treatment belongs with your prescriber or doctor, who knows your full medical history.

Frequently asked questions.
The black triangle (▼) means Mounjaro is under additional monitoring to build up safety data quickly. It does not mean the medicine is unsafe. It means any side effect you notice is worth reporting to your national reporting scheme and discussing with your prescriber or doctor.
A precautionary thyroid warning exists based on animal studies. Whether the risk applies to humans is not established, but Mounjaro is generally not recommended for people with a personal or family history of medullary thyroid carcinoma or MEN 2. Discuss any thyroid history with your prescriber or doctor.
Severe, persistent abdominal pain — particularly if it radiates to the back — with or without vomiting can be a sign of pancreatitis. If this happens, contact a healthcare professional promptly rather than waiting to see if it resolves on its own.
Mounjaro alone carries a low hypoglycaemia risk. However, if you also take insulin or a sulphonylurea, the combined risk is higher. Your prescriber or doctor should guide you on monitoring — never adjust your other diabetes medicines without their advice.
Logging symptoms in a tracking app like GLP Trackr lets you record what you experience after each dose and see patterns over time. This gives your prescriber or doctor a structured, accurate record to work from rather than relying on memory alone.
